YouTube thumbnail size
A YouTube thumbnail should be 1280 by 720 pixels, a 16:9 ratio. That is the size YouTube recommends and the one every player size scales down from cleanly.
The minimum accepted width is 640 pixels, but do not aim there. Your thumbnail gets shown at everything from a tiny sidebar tile to a full-width home page banner, and a 640-wide image looks soft on the big ones. 1280 by 720 costs you nothing and covers every placement.

The 2 MB limit
YouTube rejects custom thumbnails over 2 MB. This is the single most common reason an upload fails, and it usually happens to people exporting a PNG from a design tool.
The fix is almost always the format. A 1280 by 720 photo saved as JPG lands somewhere around 100 to 300 KB. The same image as PNG can run past 2 MB on its own. Save JPG unless your thumbnail is flat graphics or text on a solid color, where PNG stays sharp and small.
If you are still over after switching, compress it to 500 KB and you will be well clear with no visible loss at this size.
Accepted formats
JPG, PNG, GIF, and BMP are all accepted. WebP is not accepted as a custom thumbnail even though YouTube serves WebP itself, so convert before uploading.
Custom thumbnails also require a verified YouTube account. If the upload button is missing entirely rather than failing, that is the reason, not the file.
Keep text away from the corners
The bottom right corner of every thumbnail gets covered by the video duration stamp. Anything you put there is hidden on every surface where the video appears.
The rest of the safe-zone advice is simpler than people make it. Thumbnails get displayed as small as about 120 pixels wide in sidebars and search suggestions. Whatever text you add has to survive that, which in practice means three or four large words, not a sentence. Channel art is the awkward one. The full 2560 by 1440 image is only ever shown on a TV. On desktop you see a wide strip, and on a phone even less, so everything that matters has to sit inside the central 1546 by 423 safe area.
